Request for Continuing Education Credit Hours
Contractors who attend a Blue Stakes of Utah Damage Prevention Awareness Seminar are eligible to receive two continuing education credit hours with verification of attendance toward their contractor licensing as required by the State of Utah’s Division of Occupational and Professional Licensing (DOPL) as follows:
CONTRACTOR (2 core credit hours) – License Number ending with 5501, 5551
PLUMBER (2 professional credit hours) – License Number ending with 5508, 5509, 5510, 5518, 5519
ELECTRICIAN (2 professional credit hours) – License Number ending with 5502, 5503, 5504, 5505, 5506
Drinking Water & Waste Water Professionals who attend a Blue Stakes of Utah Damage Prevention Awareness Seminar are also eligible to receive 0.2 CEU’s with verification of attendance towards their certification number.
If you attended a 2014 or 2015 Damage Prevention Awareness Seminar and did not request the credits at the time of attendance you may still be able to receive them by submitting the following information to Blue Stakes.

- Full Name of Attendee
- Date & Location of Seminar Attended
- All Applicable License or Certificate numbers
